There IS overlap with physiotherapy & massage but it can be confusing which to choose when you have lower back pain

Which Is right for your lower back pain?Physiotherapy or massage
Such a common question we hear in clinic.. " I've got back pain, I'm not sure whether I need to book a physio or a sports massage session ? " Whilst both treatments have their place when i comes to treating lower back pain, understanding the differences between them can help you make an educated decision about which option is best for your specific symptoms.
Physiotherapy vs. Massage: Key differences
A sports massage therapist is trained in soft tissue techniques, typically through a series of weekend courses progressing through levels 1-5. While they are skilled in relieving muscle tension and promoting relaxation, they are not qualified to diagnose or treat underlying medical conditions or injuries. Conversely, physiotherapists and sports therapists complete a graduate university degree and can specialise in diagnosing, treating, and preventing musculoskeletal issues. Physiotherapists training includes experience in neurological, cardiac and respiratory areas and so they have a wider variety of techniques in their repertoire. This makes them better equipped to assess and address the root cause of your back pain, especially if it’s a new or severe issue.
The choice between physiotherapy and massage ultimately depends on the nature and severity of your back pain.
When to choose physiotherapy for lower back pain
If your back pain is moderate to severe, or if it’s a new issue, physiotherapy ( or sports therapy ) is often the best choice. They will conduct a thorough assessment to identify and diagnose the cause of your pain and create a personalised treatment plan for you. This plan may include exercises, manual therapy, acupuncture and education to help you recover and prevent future episodes.
Physiotherapy is a more holistic approach which requires active participation from the patient. To address the underlying issue and achieve long-term relief you will need to do your homework !
When to Choose Massage for Lower Back Pain
Massage therapy, particularly deep tissue or sports massage, can be beneficial for managing mild back pain or recurring tightness caused by factors like regular exercise, desk work, a heavy job or stress. If you tend to hold tension in your back or experience discomfort from prolonged sitting, massage can provide short-term relief by relaxing tight muscles and improving circulation.
However, it’s important to note that massage alone may not resolve the root cause of your pain, especially if it’s related to other physiological or psychological factors. While it can be a helpful part of your self-care routine, it’s not a substitute for professional diagnosis and treatment when needed.
Causes and Symptoms of Back Pain
We're wired up ina funny fashion and back pain can stem from a variety of causes, including nerve irritation or compression, muscle strain, or even referred pain from other distant areas of the body. For example, leg pain, pins and needles, or numbness may indicate nerve compression in the lower back. In such cases, physiotherapy is the recommended initial approach to address the underlying issue and get a correct diagnosis.
There are also rare but serious symptoms that require immediate medical attention. If you experience any of the following, consult a doctor right away:
Difficulty passing urine or feeling you need to go when the bladder is empty
Numbness or tingling in the genitals or buttocks
Loss of bladder or bowel control ( incontinence )
Impaired sexual function, such as loss of sensation during sex
Weakness or feeling of floppiness in the legs
Pain radiating down both legs
Feeling unwell and / or fatigued with back pain, such as fever or night sweats
Which Treatment Is Right for You?
The choice between physiotherapy and massage ultimately depends on the nature and severity of your back pain. If you’re dealing with a new or persistent issue, physiotherapy is the best option for diagnosis and long-term recovery. For mild discomfort, tension relief or training recovery massage can be a helpful therapy.
